- DTaP – 5 doses
- Polio- 4 doses
- Hepatitis B- 3 doses
- Measles, Mumps, and Rubella (MMR)-2 doses
- Varicella (Chickenpox)- 2 doses
- Tdap – 1 dose 7th Grade entry
