If you are interested in enrolling your student in National City Middles Schools Lion’s Den Before and After School Program, please fill out the Google Form (link below).  Once we receive the form an email will be sent to you with a link for the APPLICATION. You must follow the directions on email in order to enroll your student into the program. The email heading will read “Program Enrollment for Sweetwater National City Middle School”. You will be provided with a link to enter a username and password to start the application. We will confirm registration once you complete the registration form. YOUR CHILD IS NOT ENROLLED UNTIL THEN.
